New Patients
At Progressive Pediatric Therapy, we believe that the child’s parents and family have the primary role in encouraging their child’s development.
What is needed for my first visit?
We kindly ask you to fill out the Intake form 24 hours prior to your appointment. Forms will be emailed to you from IntakeQ. In the event you did not receive the form, please check your spam folder and/or contact our clinic.
Please arrive 5-10 minutes prior to your appointment, so that we can explain the sign-in process, and what to expect. You may wait in our lobby for your therapist (please wear a mask) or upfront by the clinic door.
Evaluations require a parent or caregiver present. A maximum of two adults are permitted. For the safety of our patients and therapists, we ask you to please wear your mask at all times. Please bring a pair of socks, we will ask you and your child to remove your shoes when entering the therapy session.
